I hereby authorize PURE* to change my autopay detail as above and debit all amount due for services under my or Transferee’s (if applicable) account from the credit card account given above. In the event of the bill being outstanding for over 30 days, please charge the above credit card account (if so provided) to settle the relevant payment.

I agree the personal data and credit card data contained by this form will be collected and processed by PURE* and can be disclosed to the credit card-issuing bank/company, where deemed necessary, for the purpose of processing the change autopay detail form.
